If you have other sessions running with same username/password, it could still be a problem on ISP end. Radius can be configured to allow only certain number of sessions and reject if it crosses the limit. This is just one possibility and only ISP can confirm if that is the case. However if you are able to disconnect any existing session and try if this session comes up during that time, it will confirm the cause and you can take it back to the ISP. But yes it will be disruption to the existing session, you may take a call if there is not much support from ISP.
